The evolution of nuclear medicine now enables clinicians the ability to offer patients increasingly customized personalized care. Theranostic pairs of radiopharmaceuticals typify this change. Diagnostic radiopharmaceuticals can assist physicians in diagnosing or staging a disease. Following this, physicians can then customize a treatment plan using a therapeutic variant of the same radiopharmaceuticals with identical mechanisms of localization. Due to the complex regulatory and operational considerations involved in using theranostic radiopharmaceuticals, a multidisciplinary approach is warranted when establishing a program. Theranostic programs frequently involve infrastructure and radioactive material license changes.

  • Define theranostic nuclear medicine and review its historical perspective and current applications.
  • Discuss opportunities and challenges associated with handling theranostic radiopharmaceuticals.
  • Discuss key operational considerations when establishing a theranostic program.
  • Discuss several regulatory considerations when establishing a theranostic program including: RAM licensing, equipment, shielding, and facility requirements.
